Main item's material: cardstock

Main item's features: folded at the center, design on both sides, square corners and placeholder data already present as shown in the image on the left.

Main item's description: immunization record card issued by the Department of Defense (DOD) to military personnel for medical purposes pursuant to Article 99 of the International Sanitary Regulations.

Remarks: the "1953 Edition" and the "1964 Edition, Series 1" feature the same font style for the words "UNITED STATES OF AMERICA DEPARTMENT OF DEFENSE" and smaller texts (E.G.: "CERTIFICAT DE VACCINATION"), the same design for the DOD seal and display the same serial number at the bottom of the backside/inside of the card, while the "1964 Edition, Series 2" displays a slightly different font style for the above stated text, a different design for the DOD seal and lack the serial number in the above stated position. The "1953 Edition" features a field for "Race" while the field is not present in any of the "1964 Edition" series. The "1953 Edition", the "1964 Edition, Series 1" and the "1964 Edition, Series 2" feature different fonts and texts for the form number area (front side/outside, below the DOD seal area). Both the "1953 Edition" and the "1964 Edition, Series 1" feature 8 lines for the "Cholera" and "Tetanus" sections (backside/inside of the card) while the "1964 Edition, Series 2" features 7 lines for the same sections. On the "1964 Edition, Series 2" the text in italic is slightly smaller than on the "1953 Edition" and "1964 Edition, Series 2" certificates.
